Official Contest Rules

1.     The Columbus Radio Group will award two [2] roundtrip airline tickets from Columbus, Ohio to any regularly serviced destination by Air Tran Airlines in the Continental U.S. (48 states); totaling approximately $1,000 value.

How to win: People ages 18+ will qualify January 1, 2009-December 31, 2009 at anyone of these events below:

2.    At anyone of the above events; stop by each of the event sponsor booths and get your “Passport” stamped or signed by that sponsor representative. Complete the questionnaire. Deposit the completed questionnaire into the baskets at the exit locations. You may also register by sending a postcard or envelope with your address, and phone number (day/evening) to: “Passport Fly-Away”, c/o Sunny 95, 4401 Carriage Hill Lane, Columbus, Ohio 43220. Only one entry per person will be accepted.
*All entries must be postmarked and received by December 31, 2009. Sunny 95 is not responsible for damaged or lost entries. Random drawing to take place Monday, January 4, 2010 from all entries received.

3.     Winner will be notified by telephone. The Columbus Radio Group cannot be responsible for a person’s inability to
      complete the required entry. Odds of winning are determined by the number of entry forms received.

4.     Contest is open to any resident of Columbus and the surrounding listening area over the age of 18, except employees, their families, officers, directors and stockholders of our Columbus Radio Group event sponsor. The Columbus Radio Group reserves the right to disqualify any winner if the contest rules have been violated. Contest void where prohibited.

5.     Winner Release: Prize winner must present identification and sign a release a)permitting reasonable use of name, voice, likeness, and biographical information and b)releasing The Columbus Radio Group, any of the participating sponsors, their officers, directors, stockholders and employees from any and all claims, liabilities and damages arising directly or indirectly out of award or use of the Prize. In accordance with IRS regulations, any winner receiving Prizes(s) valued at $600 or more in a given calendar year will be furnished a copy of IRS Form 1099 (miscellaneous income). The winner agrees to waive any claim of royalty or right of remuneration for any such promotional use and agree not to appear on, or on behalf of, any other radio or television station for a period of one (1) year after acceptance of Prize.

6.     Prize to be claimed at the Sunny 95 offices (8:30am-5:30pm Monday-Thursday and 8:30am-7pm Wednesday). Winner is required to present a valid article of identification as described above at this time. All identification submitted to verify a winner is subject to acceptance by The Columbus Radio Group in its sole discretion. Identification which is counterfeit, illegible, tampered with or otherwise altered will not be accepted. Prize must be claimed by Friday, January 22, 2010 at 5:00pm. The Prize not claimed within the allotted time period shall remain the sole property of The Columbus Radio Group. The 1-year waiting period to play another major contest is in effect.

7.     The person who enters to win, claim the Prize and the person redeeming the Prize must be one and the same, and will be considered the sole and exclusive winner. Prize is non-transferable and non-assignable. Winner will be obligated to sign and return an Affidavit of Eligibility and provide their social security/tax identification number before receiving the Prize. Failure to comply with any requirement of these terms and conditions will result in the winning entrant being deemed ineligible for award. Prize winner is responsible for any federal, state and local taxes as well as any additional expenses incurred with use of the Prize.

8.     No purchase necessary.

9.     Where applicable; The Columbus Radio Group, affiliates, participating sponsors, their agencies and the like, or any member thereof, make no claims of merchantability or validity of Prize. See Airline for complete warranty as it pertains to Prize.

10.    All decisions of The Columbus Radio Group concerning this contest or eligibility are final.

11. In the event the winner is subject to or the subject of any order or legal process issued by any governmental
      agency having jurisdiction over the affairs of the winner, (e.g. garnishment, child support order, judgment,
      lien and the like), The Columbus Radio Group delivery of the Prize to the official representative of the
      governmental entity claiming a right to the Prize shall be entitled to rely in good faith upon any documents
      presented by the representatives seeking to collect the Prize in lieu of the winner. Presentation of the Prize
      to such authorized representative or entity shall be deemed same as having given the Prize directly to the
      winner. The Columbus Radio Group shall not be liable for any claim by winner for damage incidental thereto.

12.    Official contest rules are available at Sunny 95 offices during normal business hours, (8:30am-5:30pm Monday-Thursday and 8:30am-7pm Wednesday) or on-line at Sunny95.com/contest rules. Rules are subject to change without notice. The Columbus Radio Group reserves the right to postpone or extend the contest dates.
